Any contractor wishing to do business in the Town of Firestone must complete one of the following forms. To see which Class applies, see the Contractors License Information Sheet below.

The Town of Firestone requires proof of workers compensation and general liability insurance showing the Town as additional insured

Please fax your insurance as soon a possible, and have your agent send us an original copy.

If we do not receive your current insurance we will not be able to issue your contractors license.
